How to Become a Client

Woolwich Community Health Centre is not currently accepting new clients.

To help you find a family doctor you can register with Health Care Connect.

Please note, patients cannot “double-doctor” for primary health care. You cannot become a registered client at WCHC if you are already registered with another primary care provider.

 

How to Make an Appointment

You can book an appointment by calling the reception desk during regular hours of operation.

When you call, reception or nursing staff will assess your health needs and book an appropriate appointment. Please book well ahead for non-urgent concerns. For example, prescription renewals or preventive health screening. Health care providers will make home visits for clients who cannot get to the health centre for reasons related to their medical condition.

 

Physiotherapy Clinic – How to Make an Appointment

Registered clients who do not have physiotherapy coverage through 3rd party insurance or have exhausted their insurance benefits, can be referred by their primary care provider to the WCHC  physiotherapist. Or, you can self-refer by calling the physiotherapy clinic at 519-664-3794 ext. 286.

 

Lab Services

Lab services are offered by appointment only at our St. Jacobs and Wellesley locations to our registered clients.

Prescription Renewals

Prescription renewals require an appointment with either a physician or nurse practitioner. If you take regular prescription medication, remember to book an appointment to have your medication(s) renewed at least 2 weeks before you run out. Additionally, bring all your medications with you to every appointment to ensure that your provider can refill all your medications as needed.

 

After Hours Care

Call 1-866-933-0212 to reach our after hours answering service if you require urgent health care advice outside of our regular hours. The answering service will contact the on-call physician or nurse practitioner. Then, the provider will call you back to discuss appropriate care.

 

Hospital Discharge

If you have been recently discharged from the hospital, please book a follow up appointment with your provider as soon as you are able. Your provider will follow your recovery and provide you with resources to aide you in your return to your regular daily activities.
