We are pleased to offer our registered clients a convenient and efficient way to schedule appointments with our dedicated healthcare providers. Our online booking system allows you to manage your healthcare on your terms, providing easy access to book appointments.

Please note:

• This booking platform is only available for patients rostered at our clinic, if you are not a patient at our clinic but are interested in becoming a patient please contact us at 519-656-9025  Ext. 297
• You will need your valid provincial health card and the name of your provider to book an appointment using this platform.
• Please remember to bring your valid provincial health card with you to your appointment.
• If you need to reschedule or cancel your appointment, please call the clinic.
• If you do not know the name of your provider, please call the clinic.
• If you are experiencing any acute or urgent health concerns, please contact our office by phone to book your appointment.
• If your concern is an emergency, please call 911 or go directly to your nearest emergency department.

¡Bienvenido al sistema de reserva de citas en línea del Woolwich Community Health Centre!

Es un gusto poder ofrecer a nuestros clientes registrados una manera eficiente de programar citas con nuestros proveedores medicos. Nuestro sistema de reservas en línea le permite reservar cita médica según sus propios términos.

Tenga en cuenta:

• Esta plataforma de reserva solo está disponible para pacientes registrados en nuestra clínica, si no es paciente de nuestra clínica pero está interesado en ser paciente, contáctenos al numero 519-656-9025 ext. 297
• Necesitará su tarjeta sanitaria provincial vigente y el nombre de su proveedor para reservar una cita a través de esta plataforma.
• Recuerde traer su tarjeta sanitaria provincial vigente a su cita.
• Si necesita reprogramar o cancelar su cita, llame a la clínica. Si no sabe el nombre de su proveedor, llame a la clínica.
• Si tiene algún problema de salud agudo o urgente, comuníquese con nuestra oficina por teléfono para programar su cita.
• Si su inquietud es una emergencia, llame al 911 o vaya directamente al departamento de emergencias más cercano.
